Initiatives that Support Richmond Family Physicians
The Richmond Division of Family Practice leads and supports initiatives that strengthen family practice, primary care, and community health. Through the Richmond Primary Care Network, the Long-Term Care Initiative, the Richmond Attachment Program, and the MOA Network, we work with family doctors in Richmond and their teams to improve patient care, connect unattached patients, and build a stronger, more supportive health system.
Richmond Primary Care Networks (PCN)
The Richmond Primary Care Networks (PCN) bring care teams together to focus on each patient’s health. Residents also get access to free services like physiotherapy, counselling, and more.
Richmond Attachment Program (RAP)
The Richmond Attachment Program (RAP) connects unattached vulnerable patients in Richmond, like moms, complex care, and newcomers, with family physicians who have capacity to accept new patients.
Long Term Care Initiative (LTCI)
The Long-Term Care Initiative (LTCI) empowers Richmond family physicians to create local, scalable solutions that improve patient care in long-term care homes. This is an exciting opportunity to make a lasting impact in your community.
