Stronger Health Outcomes Through Patient Connection: Long-Term Care Initiative

The Long-Term Care Initiative supports Richmond's long-term care homes through physician recruitment, quality improvement efforts, and ongoing physician support, helping strengthen workforce sustainability and continuity of resident care.

The Long-Term Care Initiative (LTCI) had a stable and productive year, supported by a growing team of dedicated long-term care physicians. The program expanded from 18 physicians in 2024 to 23 physicians in 2025, now providing care across six Richmond care homes:

  • Rosewood Manor
  • Fraserview Retirement Community
  • Pinegrove Place
  • Richmond Lions Manor
  • Minoru Residence
  • Hamilton Village Care Centre

This growth reflects the successful onboarding of Hamilton Village Care Centre and the recruitment of two new physicians. Each care home is supported by a collaborative team of LTC physicians and dedicated care-home staff committed to delivering high-quality resident care.

Quality Improvement Initiatives for the Long-Term Care Initiative

This year’s work focused on quality improvement projects developed in partnership with Vancouver Coastal Health Long-Term Care, including:

  1. Streamlining physician onboarding by improving pathways for return-of-service physicians to join long-term care without delays.
  2. Strengthening communication between LTC physicians and care-home operations teams.
  3. Improving access to clinical information by enabling after-hours PointClickCare access for all LTC physicians, supported by training and tutorial resources.
  4. Establishing performance measurement frameworks, including key performance indicators (KPIs) and evaluation processes for the Richmond LTCI.
  5. Sharing successful practices by presenting RDFP recruitment strategies and KPI frameworks with Doctors of BC and other Divisions of Family Practice.
  6. Enhancing care coordination by encouraging non-LTCI physicians to transfer residents to LTCI physicians or provide clear daytime and after-hours contact information for urgent care needs.
  7. Expanding physician recruitment efforts by promoting LTC opportunities through RichmondHealthcareJobs.ca, Health Match BC, and Vancouver Coastal Health Physician Careers.

Richmond LTCI Looking Ahead

Through strategic recruitment, collaborative quality improvement initiatives, and strengthened physician support, the LTCI continues to build a sustainable long-term care workforce while improving continuity of care for residents across Richmond’s long-term care homes.
